Scholarship Payment Form - Woodrow Wilson High School Community Foundation

Per IRS guidelines, we can only mail the scholarship check directly to your school. To send the check, we need the following information:

Scholarship Awarded

Student ID# (College)

Student Email Address

Name of School

School Mailing Address for Outside Scholarships

(often it is the address of the financial aid office)

Street Address Address Line 2 City State ZIP Code

Proof of Enrollment

Examples of acceptable documentation include: class schedule for the current or upcoming semester, an invoice or receipt (for any direct charges by the school), verification of enrollment form signed by a school representative
